We are currently accepting applications for the 2021-2022 school year. If you would like to enroll your child, please complete the following items. You may drop forms off at our school, or mail them to GLMS, PO Box 903, Midway, Utah 84049.

  • Enrollment Agreement
  • Annual Registration Fee of $300 by check to GLMS
